Care Management
Care Management is an essential and valuable component of a Home Care Package. Our Client Service Managers will ensure your care and services are coordinated and aligned with your individual goals and needs.
As you grow older, living independently in your own home may become more difficult. Help at home looks different for each person and there are a variety of services available to suit your needs.
Funding
Entry-level care – Commonwealth Home Support Programme (CHSP)
If you’re able to manage at home, however require support with a few tasks, you may be eligible for subsidised support services through CHSP.
The program aims to help people live as independently as possible with low level support. Most recipients of CHSP only require one or two services to help them stay independent.
More complex care – Home Care Packages
If you require care and support services on an ongoing basis, you may be eligible for a Home Care Package. These packages are for those with more complex care needs.
Home Care Packages provide a number of services to meet your needs.
Specialised Care Services in Launceston
If you or your loved one has more complex needs, our specialised care services can assist you to remain living safely and comfortably at home.
In-Home Dementia Care
The familiarity of home provides immense security to those living with dementia. Being surrounded by personal belongings and memories offers great comfort, and helps to improve awareness and reduce stress. Our dedicated carers at Regis provide personalised care and support to our clients living with dementia.
In-Home Palliative Care
Many people needing palliative care prefer to be cared for at home where they feel most at ease. It’s essential to our palliative care team that our clients are able to enjoy the independence and comfort of their own home, while also receiving proper medical assistance for a better quality of life.
24 Hour In-Home Care
For each of us, there may come a time when a friend or family member needs around the clock care. We provide personal care services all throughout the day and night to our clients, so they receive the right level of support for their needs, and their loved ones are afforded peace of mind whilst they take a break.
Changing Care Needs
Our Day Respite Centre is also located next to Regis Legana residential care home, so if you or your loved one ever need increased short- or long-term assistance, we can provide an easy and supportive transition.
Respite Care in Legana and Launceston
Our respite services enable those caring for their loved ones the ability to take much-deserved downtime, as well as the reassurance that their friend or family member is being well cared for.
Centre-Based Respite in Legana
We offer social support groups at our Day Respite Centre in Legana, where our group members enjoy activities and outings in the company of others.
Our groups participate in a range of activities such as puzzles and games, arts and crafts, theme days, guest performers, and discussing the local news and general life. From time to time we also do outings and day trips around Launceston and the local area.
Transport to and from groups, and meals (morning tea, lunch, and afternoon tea), are included.
